Care Coordination: Professional support when it matters most

Care Coordination is for children and youth who live in St. Johns County and are connected with any child welfare system - foster, adoption, child protection, family support services, and courts.

Care coordination can be helpful when:

  • Your child has had frequent crises, big emotional reactions, or safety concerns

  • You’re juggling appointments, services, or providers and it feels hard to keep everything connected

  • You’ve tried getting help, but don’t know what service to ask for

  • Your child is going through a big change, like a new school, placement changes, or returning homefrom a hospital or program

how we help families - a care coordinator works alongside you, not to judge you or tell you what to do, but to help:

  • understand your child’s needs and strengths

  • connect you to the right supports and services

  • communicate with schools, providers, and other agencies

  • plan for challenges before they become a crisis

  • make sure help is available 24/7 through the Mobile Response Team
