Care Coordination is for children and youth who live in St. Johns County and are connected with any child welfare system - foster, adoption, child protection, family support services, and courts.
Care coordination can be helpful when:
Your child has had frequent crises, big emotional reactions, or safety concerns
You’re juggling appointments, services, or providers and it feels hard to keep everything connected
You’ve tried getting help, but don’t know what service to ask for
Your child is going through a big change, like a new school, placement changes, or returning homefrom a hospital or program

how we help families - a care coordinator works alongside you, not to judge you or tell you what to do, but to help:
understand your child’s needs and strengths
connect you to the right supports and services
communicate with schools, providers, and other agencies
plan for challenges before they become a crisis
make sure help is available 24/7 through the Mobile Response Team
